Using the services of a Private Health Advocate gave me a tremendous amount of peace of mind when my father's medical condition took a rapid change for the worse about a year ago. Knowing that a registered nurse was going to Dad's doctor visits and overseeing his medications was very reassuring. As I lived over an hour and a half away, it was not always easy to drop in. Heartwood Health also helped clarify things with Dad's long term care insurance so that some of the cost would be covered and set up emergency backup care in the event that Dad might become ill while I was out of the area. Their services helped Dad stay at home safely. They lifted such a weight off my shoulders that I have already recommended them to two of my personal doctors as well as to several friends. Our Private Health Advocates will:
- Provide education and support
- Perform general assessment of our clients needs and develop a plan of action
- Coordinate discharge planning with staff, outside agencies and family members
- Screen, arrange and monitor in home health or other services
- Facilitate and/or attend team meetings with medical staff
- Assist with identifying appropriate placement in an assisted living or nursing home
- Provide ongoing monitoring and communication for long distance caregivers
- Review and interpert insurance policies, including medical, pharmacy, long term care and disability plans
