A Private Health Advocate (PHA) also known a patient navigator will support and assist you and your family with any aspect of care from birth to the end of life. Our PHAs are trained and experienced in nursing, social work, psychology and pharmacy. They are all licensed healthcare professionals and have worked in a variety of healthcare settings. All of our PHAs have personal experience advocating for family and friends and bring extraordinary passion to their work with clients.
Our Private Health Advocates will:
- Provide education and support
- Perform general assessment of our clients needs and develop a plan of action
- Coordinate discharge planning with staff, outside agencies and family members
- Screen, arrange and monitor in home health or other services
- Facilitate and/or attend team meetings with medical staff
- Assist with identifying appropriate placement in an assisted living or nursing home
- Provide ongoing monitoring and communication for long distance caregivers
- Review and interpert insurance policies, including medical, pharmacy, long term care and disability plans
