Michelle and Patti have over 35 years of combined hands-on healthcare experience. In addition to clinical work, both have been highly active volunteers in the hemophilia and bleeding disorders community in Northern California for decades. Michelle is an RN, MSN and Nurse Practioner, while Patti is a registered Pharmacy Technician and Hypnotherapist. Private Health Advocate Division

Individuals often find themselves in unfamiliar territory while trying to navigate the complexities of the health care system, and wish they had an "expert" to assist them. Heartwood Health provides this expertise through our Private Health Care Advocate Division, assisting people of all ages . We provide care coordination and options for those facing difficult decisions and challenges. Our Health Advocates are objective, non-biased professionals with years of experience in nursing, social work, gerontology, oncology, women's health, insurance and reimbursement. We provide geriatric care managers throughout the state of California and can also perform umbrella advocacy services nationwide. Our company approaches your needs from a team perspective and tailors a plan unique to each situation.

Specialty Care Division

In addition to providing Health Advocate Services to all our clients, we serve the needs of those with chronic illness, specializing in hemophilia and other bleeding disorders. Treatment and services for hemophilia have come a long way in the last twenty years. Factor replacement therapy is readily available, and with proper treatment, a person diagnosis with Hemophilia now can expect to a normal life expectancy. In addition to providing Factor Replacement products to our clients, our services include ongoing care coordination with Hemophilia Treatment Centers and other providers. We view our role as collaborative with your medical team. We provide ongoing clinical support, including superb customer service and communication about your health care needs.
